The Top 100 Drugs in Sterile Compounding — Part 1: Anti-Infectives

Welcome to a four-part series built for the technicians who work — or want to work — in the IV room. The short version: IV anti-infectives are the workhorse of most sterile compounding operations. These are the antibiotics, antifungals, and antivirals reconstituted and admixed into IV bags every shift. Here are 25 of the most common — organized by class.
Beta-lactams (penicillins, cephalosporins, carbapenems)
- Vancomycin — glycopeptide; MRSA and serious gram-positive infections. (Technically not a beta-lactam, but one of the single most-compounded IV antibiotics — leading the list for a reason.)
- Piperacillin-tazobactam (Zosyn) — extended-spectrum penicillin + beta-lactamase inhibitor; broad empiric coverage.
- Cefazolin (Ancef) — 1st-generation cephalosporin; surgical prophylaxis and gram-positive infections.
- Ceftriaxone (Rocephin) — 3rd-generation cephalosporin; broad-spectrum, once-daily workhorse.
- Cefepime (Maxipime) — 4th-generation cephalosporin; includes Pseudomonas coverage.
- Ceftazidime (Fortaz) — 3rd-generation cephalosporin; strong anti-pseudomonal activity.
- Meropenem (Merrem) — carbapenem; broad-spectrum for resistant/serious infections.
- Ertapenem (Invanz) — carbapenem; broad-spectrum, once-daily dosing.
- Ampicillin — aminopenicillin; gram-positive and select gram-negative coverage.
- Ampicillin-sulbactam (Unasyn) — aminopenicillin + beta-lactamase inhibitor.
- Aztreonam (Azactam) — monobactam; gram-negative coverage, often used in beta-lactam allergy.
Aminoglycosides & fluoroquinolones
- Gentamicin — aminoglycoside; gram-negative infections (levels monitored).
- Tobramycin — aminoglycoside; gram-negative, including Pseudomonas.
- Amikacin — aminoglycoside; reserved for resistant gram-negatives.
- Ciprofloxacin — fluoroquinolone; broad gram-negative coverage.
- Levofloxacin (Levaquin) — respiratory fluoroquinolone; broad-spectrum.
Other antibacterials
- Metronidazole (Flagyl) — nitroimidazole; anaerobic and protozoal infections.
- Clindamycin — lincosamide; gram-positive and anaerobic coverage.
- Azithromycin — macrolide; atypical and respiratory infections.
- Linezolid (Zyvox) — oxazolidinone; resistant gram-positives, including VRE.
- Daptomycin (Cubicin) — lipopeptide; MRSA and VRE bloodstream infections.
Antifungals
- Micafungin (Mycamine) — echinocandin; invasive candidiasis.
- Fluconazole (Diflucan) — azole antifungal; candidal infections (often available premixed, also admixed).
Antivirals
- Acyclovir — antiviral; serious HSV and VZV infections. Handle with care — high pH, irritant.
- Ganciclovir (Cytovene) — antiviral for CMV. Hazardous drug (NIOSH list) — prepare per USP <800> containment.
Technician takeaway
Anti-infectives are where most IV-room technicians build their reps. Knowing the class tells you a lot about what to expect — reconstitution needs, common diluents, light sensitivity, and which agents carry hazardous-handling requirements. Master these 25, and you’ve covered a huge share of a typical day’s compounding.
